to the following conditions of our application process.
I understand that upon completion of the within application, a member of The
Boxer Rescue, Inc. (hereinafter referred to as "TBRI") will contact me within 48 hours. I understand at that time, a telephone
interview will be conducted concerning the Boxer dog I wish to surrender into the TBRI program. After said interview, if the
TBRI representative feels my dog is a candidate for its program, a TBRI member will come to my home to evaluate my dog. If it
is determined that my dog is acceptable for TBRI'S program, I agree to turn over ownership to TBRI and sign all legal documents necessary
for the surrender of my dog. It is understood that TBRI is under no obligation, at any time, to take my Boxer dog into its rescue

our rescue program you can rest assured that your dog will be provided with wonderful care by our carefully selected foster families. Once your dog is placed in a foster home, he/she will be evaluated and be given unconditional love and care. Of
course, this care will continue until your dog is matched with his/her “forever” home which will be determined by our Directors who
extensively screen all applicants through telephone interviews, reference checks and home visits.
When filling out this form,
please be honest and as detailed as possible. This will allow us to have the most accurate information concerning your dog’s
personality and temperament. This information is essential to us and your dog in many ways. It will allow the foster family
to focus on any strengths or weaknesses your dog currently has, routines and commands your dog is familiar with which makes the adjustment
easier for your dog. It also allows us an idea of any issues your dog is experiencing thus allowing the foster home
the opportunity to take the necessary safety measures within their families and most important, information that will help the new
